What’s Chronic Care Management?

Some people think chronic care management is just another provider visit. It is not. It is support to help you take care of yourself. It does not replace your provider—it works with your provider to help you stay healthy between visits.

You will work with a care team that helps you follow your treatment plan, take your medicine, and understand your health. We can also help with everyday problems like getting to appointments, finding healthy food, or accessing housing.

Conditions We Can Help You Manage

You may benefit from this program if you live with conditions like:

  • Asthma
  • Chronic obstructive pulmonary disease (COPD)
  • Diabetes
  • Heart disease
  • High blood pressure
  • High cholesterol
  • Mental health conditions like depression or anxiety
  • Seizure disorder

If you have other ongoing health issues, you may still receive help.

Do I Need Chronic Care Management?

Most people need chronic care management because they have two or more long-term health problems. You may find it hard to keep up with provider visits, medications, or understanding your condition.

If you qualify your provider may enroll you to be contacted by our Chronic Care Management team by phone.

Expect easy-to-understand information about your health and answers to your questions. If you need extra support, you’ll learn about local resources like transportation, healthy food programs, or other services to help you stay well.

How Much Does It Cost?

This program is free for many Medicare patients who have secondary insurance to cover expenses not paid by the primary insurance plan. If not, the cost is like most Medicare services and may include a small monthly copay, plus any remaining deductible.
